Distance Education Course Approval Form

As part of the course approval process, the Distance Education Course Approval Form below must be completed to request a new distance education course or update basic requirements of an existing course. New classes must be approved by the Department Chair or Advisory Committee, Division Dean, and Director of Distance & Alternative Education.

Additionally, the Vice President of Instruction must approve entire degree or certificate programs offered through distance education.
