Distance Education Course Approval Form

As part of the course approval process, the Distance Education Course Approval Form below must be completed to request a new distance education course or update basic requirements of an existing course. New classes must be approved by the Department Chair or Advisory Committee, Division Dean, and Associate Vice Chancellor of Distance Education.

Additionally, the Vice Chancellor of Instruction must approve entire degree or certificate programs offered through distance education.
