Faculty Counselor Evaluation

Counselor and Campus Dean of Student Affairs provide feedback to VP/SS&SS

  • Statement of Counseling Philosophy
  • Supporting documentation:
    •  Frequently used resources – agencies, internet, organizations, and references and;
    • Supporting documents – samples of handouts or retention tools, used in groups or individual settings.
  • Summary of evaluation forms:
    • Counseling Session (Student Evaluation)
    • Dean Observation:
      • Counseling Session (Dean’s Evaluation)
      • OR
      • Workshop Evaluation (Dean’s copy)
    • Workshop Facilitator Summary
  • Professional development summary

Final career and clinical counselor evaluation materials are submitted by the Executive Deans to the Office of Faculty Evaluation via Box.  Final academic counselor evaluation materials are submitted by the Deans to the Office of Faculty Evaluation via Box. After the Faculty Evaluation staff process the forms (ensure EIDs and signatures are included), they will be moved to the archival folder from the ACC Data Records Management Office.

Box is a secure cloud storage platform managed by ACC Information Technology (IT). Instructions are below:

    1. Log into your Box account.
    2. Open the “Archive” folder in your program/division folder and find the folder with the fiscal year.
    3. Upload all of the completed and final summary forms to the fiscal year folder.



    • There must be a correct EID on all forms.
    • All submitted materials must be signed by all parties, including counselors and their supervisors.
    • All submitted materials must be final.
    • All final materials must be submitted via Box.
After you have completed uploading all materials to your program/division “Archive” folder in Box, please notify the Office of Faculty Evaluation via

Important: If faculty signatures are missing in the Faculty Summary Evaluation Form, document the repeated efforts to get the signature and attach that document to the form for the archives. Also, email any faculty member who hasn’t signed and alert them to the fact that there will be documentation of repeated requests for signature attached to the evaluation summary form that is retained by the College.
