Distance Education Add a Section to the Schedule Form

To add a section to the schedule, please submit the form below to notify the Office of Distance & Alternative Education that a class is ready to be added to the schedule. The form will be forwarded to the Department Chair for approval. Once approved, Curriculum Services is contacted to add the section to the schedule.

Note: This form is used to schedule new courses and to request additional sections of existing classes. A Course Approval Form must already be on file for the course and delivery mode being requested through this form.
